Prior to your initial visit, you will be mailed a welcome packet that includes information regarding your appointment dates, billing information, and office directions. Also included in the packet are forms that we ask you to complete and bring with you to the initial appointment. Additional authorization forms will be signed upon checking in for your child's appointment. If you are using insurance benefits, please bring your insurance cards to the appointment. Other records that can be helpful to bring include previous evaluation reports or past school records including report cards.
